Cancellation Policy
Notice of cancellation must be made in writing to the Conference Secretariat at conference@transfusion.ca . A $60.00 (plus 5% GST) cancellation fee will apply for conference registrations cancelled until May 21, 2019. After this date, no refunds will be issued.

 Registration Policies

1. Registration fees must be paid in full before you will be admitted to the conference. All registration fees are subject to 5% GST.  All fees are listed and must be paid in Canadian dollars (CAD)
2. Advance registrations will be accepted until May 28, 2019. After May 28, 2019, registrations will only be accepted on-site.
3. Substitutions may be made until May 28, 2019 by writing to the Conference Secretariat at conference@transfusion.ca or onsite.
4. Registration fees must be paid by credit card.
5. Credit card payment must be made by VISA or MasterCard.
6. A confirmation notice detailing your registration will be sent to you immediately upon completion of this registration form. If you do not receive this confirmation within 24 hours please contact us at conference@transfusion.ca.
7. Recording may occur through the conference; by agreeing to these policies, conference attendees consent to filming, photography and sound recording as delegates, speakers or exhibitors at the conference.
8. All registration rates are subject to availability.
9. Completion of this form constitutes a binding agreement to the policies listed above.

Special Events

 One complimentary ticket to each event is included with your registration. Please indicate your attendance by answering the questions to indicate which events you will be attending.  If you would like to also bring a guest, please purchase a guest ticket for the event(s).
 

 Member Registration Codes

CSTM members are required to enter the membership discount code in the "discount code" box at the bottom right of the form in order to receive the membership rate listed above. The membership code was provided to all members the week of February 11, 2019.  For those who obtained their membership after February 11, 2019 the code will be sent with your membership confirmation. 

If you need to renew or sign up for new membership, please visit the CSTM website here; alternatively you can bundle your membership with member rate conference registration by selecting the appropriate Registration plus CSTM Membership ticket.

Students

To receive the student rate, proof of full-time enrolment must be provided. A valid student card or a copy of current course schedule will be accepted. This can be emailed to conference@transfusion.ca .  Upon receipt of proof of enrollment a discount code will be provided to reduce the regular rates to student rates.  Do not register until you have received the discount codes.
